Public Health NurseSalary may be negotiable within the range listed above, based on position requirements and successful candidate's qualifications, subject to appropriate authorization.Open to the public. This recruitment will establish an open eligible list that will be used to fill current and future Public Health Nurse positions. The eligible list established may also be used to fill positions in similar and/or lower classifications throughout the County of Orange.Deadline to apply. Under the direction of the Agency Director, a new and dynamic executive team with an organizational structure comprised of five (5) service areas - Administrative Services, Behavioral Health Services, Correctional Health Services, Public Health Services, and Specialized Medical Services - is committed to meeting the health needs of the diverse Orange County community.The opportunity. The position provides services during normal business days (Monday through Friday) and may be assigned to work evening hours. Some positions are field-based and require home visitation. Responsibilities and duties for this position include, but are not limited to, the following:Possess strong understanding of federal, state, programmatic, and Division regulations, policies, procedures and protocols to accurately inform and implement rules and program requirements to children, families, and providersProvide health education, care coordination, and case management services to clients in the home or community settingProvide education and consultation to medical providers and community partnersParticipate in community organizations representing programs to advocate for health care needs of children and adults, including the sharing of local program standards and services and pediatric and adult outcome trendsOrganize and/or participate in discussion groups or on committees to develop educational programs; research and compile material for class presentation; prepare teaching aids, deliver lectures and perform demonstrationsMinimum qualifications and core competencies. In addition to the minimum qualifications, the ideal candidate will possess the abilities and/or knowledge in the following core competencies:Technical knowledge:Strong understanding and application of case management and/or care coordination principlesInterpreting and/or implementing federal, state, local, grant or clinical program regulations or policies in a clinic, community or public health environmentUnderstanding and application of the three (3) core functions and ten (10) essential services of public healthExperience working with Electronic Medical RecordsServing as a key participant in program design, development and evaluationAbility to solicit information from clients on sensitive informationInterpreting and maintaining confidentiality of client recordsProficient in Microsoft Word, Excel, Power Point and OutlookInterpersonal skills:Working collaboratively with other programs, agencies, individuals, multi-disciplinary teams, families, communities, or other community-based programsCollaborating with people from all cultures, socio-economic levels, ethnic background, and othersEstablishing and maintaining cooperative working relationships with clients, medical professionals, peers, supervisors, and managers to achieve service goalsCommunication skills:Communicating effectively with medical professionals, community agencies, and public and private sector providersPresenting information clearly and effectively in both oral and written forms of communicationRepresenting the program to individuals, groups, and community organizationsApplying teaching principles and providing education to clients and other medical professionalsProblem solving and analysis:Collecting, analyzing, and using data metrics to support the program and track for continuous operational and outcome improvementAssessing the situation of a patient (i.e. environmental and social conditions) to identify factors in being non-compliant with medical treatmentDeveloping and implementing a plan to optimally support patient compliance and adherence to medical treatment and/or personal goals, and evaluates the outcome of the plan being administeredUnderstanding and application of quality improvement/quality assurance principlesMinimum qualifications.
